Job description

The post holder will work across boundaries with a focus on long term conditions (LTCs) includingCVRM within neighbourhood teams in South East London Integrated Care System (ICS) and Kings College Hospital and Guys and St. Thomas Hospital to: Accept case referrals, provide expert opinion on medicines issues to primary, intermediate and secondary care clinicians where appropriate ensuring a consistently high clinical service is providedto patients. Work in a collaborative manner as part of Kings College Hospital, Guys and St Thomas, Hospitalrenal, diabetes cardiovascular and pharmacy departments as well as South East London Integrated Care Board medicines team, primary care networks and LTC teams to enable the delivery of safe, effective and informed medicines optimisation in the context of patient care pathways for patients with LTCs in INTs. Use clinical and change management skills to support care pathway redesign including provision of an overview of medicines usage and transfer of care issues across multiple patient pathways (andmultiple providers and commissioners). Work with the Lead Neighbourhood Pharmacist, CVRM to lead education and development in medicines management for the multidisciplinary team. Provide expert advice to GPs, clinicians and managers to ensure that national guidance and best practice underpin medicines usage and ensure compliance with the legal framework governing medicines use to ensure patient safety.

The main responsibilities of the post holder will be: Deputise for the Principal Pharmacist and/or the Lead Neighbourhood Pharmacist CVRM as required. Act as a specialist role model and an expert in the medical management of LTCs within cardiovascular, renal and metabolic specialities. Support the leadership of clinical pharmacy services in cardiovascular, renal and metabolic specialities in conjunction with the Lead Pharmacist. Participate regularly in outpatient clinics to deliver and promote high quality, cost-effective and patient focused pharmaceutical care. Act as a pharmacist independent prescriber to optimise the care of patients within CVRM. Ensure a high level of clinical pharmacy input to the care of patients within CVRM. Providing interface and discharge liaison and support. Actively participate in multidisciplinary ward rounds, delivering and promoting high quality, cost effective, patient-focused pharmaceutical care. Provide patient and carer education with close liaison across the interface. Promote safe drug use within CVRM specialties. Development, implementation and audit of local guidelines. Supervise and train pharmacy staff. Provide education and training of healthcare professionals within and outside the Trust. Regular financial reporting, drug usage and expenditure forecasting, and drug budget management. To participate in practice and clinical research and audit. To work with the Principal Pharmacist and Lead Pharmacist, to extend the boundaries of professional practice within chronic kidney disease, cardiovascular and diabetes.
