Job Purpose/Summary

To participate in the provision of a Clinical Pharmacy Service to our member GP Practices defined within the Primary Care Network geographical location.

The Clinical Pharmacist will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management and it is expected that the following will be undertaken:

Adhering to local prescribing formulary and undertaking cost-effective prescribing including Medication optimisation.

Undertake holistic structured medication reviews reducing inappropriate polypharmacy where appropriate.

Manage long term conditions.

Management of medicines on transfer of care and systems for safer prescribing.

Manage repeat prescription authorisations and reauthorisation.

Acute prescription requests.

Contribute to reductions in medicine related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines.

Identifying and addressing medicines related issues while addressing both the public health and social care needs of patients in the GP practice(s) that make up the PCN.

Medication Review and Optimisation

Utilise the LLR prescribing formulary in order to undertake evidence based and cost-effective prescribing.

Undertake holistic structured medication reviews reducing inappropriate polypharmacy where appropriate.

Manage long term conditions e.g., Heart Failure, Hypertension, Asthma, Diabetes.

Management of medicines on transfer of care and systems for safer prescribing e.g., reviewing hospital discharge letters.

Manage repeat prescription authorisations and reauthorisation.

Acute prescription requests issuing where clinically appropriate.

Medication Reconciliation

Reconcile patients medicines following clinics or hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need post discharge, including identifying and rectifying unexplained changes and working with patients and community pharmacies.

To review medications for newly registered patients.

Medicine safety

Implement changes to medication that result from CAS/MHRA alerts, product withdrawal, and other local/national guidance.

Develop and implement systems to identify cohorts of patients most likely to be at risk of unplanned admissions and readmissions from medication.

Improve prescribing practices across the Primary Care Network and facilitate changes to medication based on up-to-date evidence-based guidelines e.g., where a drug is withdrawn, or indications change.

Using Clinical systems to ensure safe prescribing e.g., Eclipse, running clinical searches.

Medicines Management

Working within the Primary Care Network team to undertake proactive and holistic routine and structured medication reviews for all patients.

Support effective medication monitoring for all patients e.g., blood tests, high risk drug monitoring, sodium valproate prescribing audit.

Run clinics within clinical competency where appropriate to meet the needs of the Primary Care Network e.g., anticoagulant monitoring clinics, diabetic reviews, hypertension reviews.

Liaise with colleagues in community pharmacy to align and support medicines adherence.

Support the practice with any reviews of repeat prescribing policy helping to improve safety and efficiency.

Medicines Information

Provide clinical medicines expertise to both patients and general practice team (e.g., around doses, side effects, adverse events, possible alternatives e.g., around out of stocks).

Improve prescribing practice through educational support for all prescribers within the Primary Care Network.

Suggest and recommend solutions using clinical knowledge and trusted pharmaceutical sources.

Practice Targets

Supporting with prescribing initiatives set both locally (Medicines Optimisation Framework) and nationally (Quality Outcomes Framework) where appropriate.

Support practices to deliver on IIF, locally commissioned enhanced services and cost-effective prescribing.

Work with PCN team to ensure the practice is compliant with CQC standards where medicines are involved.

Support the Practice/ PCN/ Federation with any projects that may present themselves, in order to support the patient demographic e.g., Mens Health Clinic, Womens Health Hub, hard to reach patient groups.

Reviewing inappropriate medications prescribed by the practice e.g., drugs no longer advised in primary care, high-cost drugs, specialist drugs.

Collaborating Working Relationships

Work collaboratively with the practice and wider PCN team as part of a multi-disciplinary approach to patient care.

Attend relevant network and practice meetings.

Support with the requirements as outlines in the PCN DES, including but not limited to providing additional hours for Extended Access.

Education, Experience and Skills

Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council.

Previous experience in a GP practice or other pharmacy settings, post registration is desirable.

Experience of working within primary care would be desirable.

Excellent interpersonal skills.

Ability to communication and present confidently, with both small and larger groups in formal and informal settings.

Ability to work both independently and as a part of a multi-disciplinary team.

Ability to engage effectively, commanding respect from a wide range of stakeholders including clinicians, patients and the public and in particular, ensuring effective two-way communication within the Primary Care Network member practices.

As part of the requirement of a Pharmacist within Primary Care, you will be required to have completed or be willing to complete the CPPE pathway (18 months approximately).

Already hold, working towards or be prepared to complete a prescribing qualification, upon successful completion of the CPPE pathway.

Demonstrates understanding of the pharmacy role in governance and is able to implement this appropriately within the workplace.

Collaborative Working Relationships

Recognises the roles of other colleagues within the organisation and their role in patient care.

Is able to recognise personal limitations and refer to more appropriate colleague(s) when necessary.

Liaises with ICB colleagues including ICB Pharmacists on prescribing related matters to ensure consistency of patient care and benefit.

Liaises with other Primary Care Networks, GP Practices and staff as needed for the collective benefit of patients.
