• Hours: 25 per week on a rota between the hours of 8am-6:30pm, Monday-Friday

The post holder is a pharmacist working within professional boundaries as part of a multidisciplinary general practice team. They support chronic disease management, undertake clinical medication reviews, and manage complex polypharmacy in a rural community with an older demographic. The role provides expert medicines advice, supports repeat and acute prescribing systems, medicines reconciliation, safer prescribing processes, and contributes to QOF and enhanced services. The pharmacist ensures effective links with community and hospital pharmacy, improves patient outcomes, and supports workload management.

Patient-Facing Work

• Conduct long-term condition clinics (e.g. COPD, asthma) focusing on medicines optimisation.

• Undertake clinical medication reviews for practice, care home and domiciliary patients, producing recommendations for nurses or GPs.

• Provide patient-facing and telephone medicines support for queries and adherence.

• Attend MDT meetings where required.

• Undertake ad hoc project work as required

Medicines Management

• Answer medicines-related queries from practice staff, community pharmacy and patients.

• Review medicines linked to unplanned admissions and implement changes for high-risk groups.

• Reconcile medicines post-discharge, resolve discrepancies, and ensure continuity of supply.

• Signpost patients appropriately for clinical issues, results, minor ailments and LTC reviews.

Prescribing & Safety

• Manage repeat prescribing processes, reauthorisation, monitoring requirements and acute queries.

• Use risk stratification searches to identify high-risk cohorts.

• Support development of services with medicinal components.

• Analyse prescribing data to highlight risks and support decision-making.

• Undertake audits, implement MHRA alerts, product withdrawals and guideline changes.

• Monitor prescribing against RAG lists, support formulary maintenance and compliance with NICE guidance.

Education & Quality

• Provide training to the primary care team on therapeutics and medicines optimisation.

• Support CQC compliance relating to medicines.

• Contribute to public health campaigns and provide specialist medicines knowledge.

Collaborative Working

• Work effectively with colleagues across the practice, locality, community pharmacy, hospital pharmacy, PCN pharmacists and other allied health professionals.

• Recognise personal limitations and refer appropriately.

• Maintain strong links across services to support consistent patient care.

Knowledge, Skills & Experience

• MPharm degree and GPhC registration; minimum 2 years pharmacist experience.

• Understanding of common acute and longterm conditions in general practice.

• Holding an independent prescribing qualification.

• Ability to follow legal, ethical and organisational policies, involve patients in decisions, and recognise priorities and deviations from normal patterns.

Leadership & Management

• Understand pharmacy governance and contribute to practice vision.

• Support quality improvement using audits and other tools.

• Understand national priorities, resource utilisation and risk management.

• Extend service delivery within team boundaries.

Education & Development

• Act as a role model, understand mentorship, deliver teaching with supervision, and maintain CPD.

• Participate in formal education programmes and appraisal systems.

Research & Evaluation

• Critically evaluate literature, identify evidence gaps, generate practice-level evidence, and apply research findings appropriately.
