Job description
The
following are the core responsibilities of the PCN Care Co-ordinator
Medicines Management.
Support
the practice medicines management programme.
Co-ordinate
routine medication reviews by identifying patients due for review and arranging
appointments.
Assist
with monitoring patients prescribed high-risk medicines in accordance with
practice protocols.
Identify
patients requiring blood tests or clinical monitoring before repeat
prescriptions can be issued.
Support
the Clinical Pharmacist and GPs with medicines optimisation initiatives.
Maintain
accurate medication records within the clinical system.
Process
administrative tasks relating to medication changes following hospital
discharge or outpatient letters.
Ensure
medication changes are actioned promptly following clinical review.
Assist
with medicines reconciliation after hospital discharge.
Monitor
prescribing alerts and ensure appropriate follow-up
Be
a single point of contact for patients, carers and families in order to help
people to manage their needs through answering queries, making and managing
appointments, and ensuring that people have good quality written or verbal
information to help them make choices about their care;
Be
a single point of contact for local community pharmacies;
To
work with the Clinical Pharmacists and Pharmacy Technicians in a supportive
role.
Accurately
document all patient contacts.
