Job description
The following are the core responsibilities of the Clinical Pharmacist. There may, on occasion, be a requirement to carry out other tasks depending on workload, staffing levels and the needs of the practice.
• Conduct structured medication reviews and repeat medication reviews for patients within the practice
• Undertake planned medication reviews for patients with long term conditions, including cardiovascular, renal, metabolic and diabetes related conditions
• Manage medication and prescribing queries and provide appropriate clinical advice
• Review hospital discharge letters and clinic correspondence, ensuring appropriate medication changes are implemented
• Reconcile medicines following patient discharge from hospital or attendance at outpatient clinics
• Review medications for newly registered patients where required
• Promote safe, effective and cost efficient prescribing within the practice
• Review medication safety alerts and ensure appropriate actions are identified and completed
• Provide support and expertise on medication monitoring and help maintain robust monitoring systems
• Carry out targeted medication reviews for patients taking high risk medicines, including DMARDs where appropriate
• Reauthorise repeat medication within your scope of practice and in line with practice procedures
• Identify patients suitable for repeat prescribing and liaise with GPs and other clinicians where required
• Support the development and implementation of safe and effective repeat prescribing systems
• Support safe prescribing and medicines optimisation for vulnerable patients
• Provide advice and clinical support to GPs, nurses, the existing Clinical Pharmacists and the wider multidisciplinary team regarding medicines management and prescribing
• Contribute to medicines searches, audits, quality improvement and CQC related work
• Support the practice in achieving relevant QOF targets and other primary care objectives
• Work collaboratively with the two existing Advanced Clinical Practitioners who are pharmacists, GPs, nurses and the wider practice team
• Use SystmOne effectively to document clinical activity, medication reviews, prescribing decisions and patient records
• Contribute to clinical governance, medicines optimisation and the ongoing development of pharmacy services within Grove Village Medical Centre
• Work within your scope of practice and use Independent Prescribing skills where appropriately qualified
• Where required, support the practice with other appropriate medicines management duties based on the needs of the service
