Job description
You will work autonomously and as part of a multidisciplinary team to identify patients at risk of medicine-related harm, optimise treatment and reduce avoidable hospital admissions. You will develop and maintain repeat prescribing procedures and searches within SystmOne, support long-term condition clinics and provide specialist pharmaceutical advice to healthcare professionals and patients. The role includes contributing to clinical governance and medicines management committees, developing policies and procedures, completing audits, supporting public health initiatives and implementing improvements arising from national guidance and medicines safety alerts. You must hold a Masters degree in Pharmacy or equivalent and be registered with the General Pharmaceutical Council . You will have relevant clinical pharmacy experience, strong clinical reasoning and communication skills, and a commitment to continuous professional development. Experience within prison, GP, hospital, community, mental health or substance misuse services would be advantageous. The successful candidate will be required to complete an enhanced DBS check and prison security vetting.
