Job description

Your responsibilities will include:

• Supporting Clinical Pharmacists and Pharmacy Technicians with medicines optimisation projects.

• Contacting patients to arrange blood tests, medication reviews and follow-up appointments.

• Liaising with community pharmacies, hospital teams and GP practices to resolve medication-related queries.

• Updating and maintaining accurate patient records.

• Supporting medicines reconciliation following hospital discharge.

• Assisting with prescribing initiatives, audits and service improvement projects.

• Helping to improve repeat prescribing processes and patient safety systems.

• Supporting delivery of Quality and Outcomes Framework (QOF) targets and other enhanced services.

• Communicating medicines-related information to practice staff and healthcare professionals.

• Attending team meetings and contributing ideas to improve patient care and service delivery.

• Promoting the safe, effective and cost-efficient use of medicines.
