Job description

The Clinical Pharmacist will work as part of the multidisciplinary primary care team, providing expert support on medicines optimisation, prescribing and patient care.

Key responsibilities include:

• Conduct structured medication reviews and support patients with long-term conditions.

• Review and optimise prescribing to ensure medicines are safe, effective and cost-effective.

• Identify and manage medication-related risks, interactions and side effects.

• Support the management of chronic conditions such as diabetes, hypertension, cardiovascular disease and respiratory conditions.

• Provide advice to GPs and other clinicians on prescribing and medicines management.

• Respond to medication queries from patients and healthcare professionals.

• Undertake medication reconciliation following hospital discharge and care transitions.

• Support audits, clinical reviews, prescribing quality improvement and QOF-related work.

• Promote safe prescribing and adherence to local and national prescribing guidelines.

• Work collaboratively with GPs, nurses, HCAs, reception and other members of the practice team.

• Maintain accurate clinical records and work within professional, legal and NHS requirements.
