Key Responsibilities:
As an Independent Prescribing
pharmacist within North Care network, you will be responsible for undertaking Patient-Facing
and remote Clinical Medication Reviews. You will lead and coordinate medicines
optimisation services and clinical pharmacy development within the PCN to
improve quality of care, personalised care and patient experience. Provide
clinical leadership to ensure high quality and cost-effective prescribing.
You will be involved in Medicines
Quality Improvement, such as undertaking clinical audits, reviewing risk of
harm, identify, plan, organise, and co-ordinate the implementation of medicines
optimisation initiatives.
The role also encompasses elements of
medicine safety, leadership and management, effective communication, drug
monitoring, repeat prescribing, service development and information management.
