Job description
Job Purpose
To support the Pharmacy Team and wider clinical team in delivering safe, effective and efficient medicines management services. The post holder will contribute to medicines optimisation by supporting repeat prescribing, medication reconciliation, prescribing administration, prescribing safety initiatives and medicines-related quality improvement activities.
Key Responsibilities
• Support the Pharmacy Team with medicines optimisation activities.
• Process repeat prescription requests in accordance with practice protocols.
• Undertake medication reconciliation following hospital discharge.
• Maintain accurate patient medication records.
• Identify patients requiring medication reviews and refer them appropriately.
• Respond to routine medicines-related queries within the scope of the role.
• Liaise with community pharmacies, hospitals and other healthcare providers regarding medication issues.
• Support prescribing audits and quality improvement initiatives.
• Assist with prescribing safety alerts, monitoring and patient recalls.
• Promote safe, effective and cost-efficient prescribing.
• Maintain confidentiality and comply with GDPR, NHS policies and GPhC professional standards.
• Participate in mandatory training, annual appraisal and continuing professional development.
• Undertake any other reasonable duties appropriate to the role.
