Job description
• The post holder will work within the specialist area, providing a high-quality clinical pharmacy service in accordance with departmental standards and current legislation.
• You will collaborate with Lead Pharmacists, clinicians, and nursing staff to optimise medicines use and improve patient outcomes.
• Key duties include medicines reconciliation, pharmaceutical care planning, drug therapy review, discharge counselling, and participation in multidisciplinary ward rounds.
• You will contribute to developing and implementing clinical protocols, ensuring safe and cost-effective prescribing in line with Trust formulary and policies.
• The role involves supporting clinical governance activities, audits, and service improvement projects.
• You will also deliver education and training to pharmacy, medical, and nursing staff, act as an educational supervisor, and maintain professional competence through CPD. Participation in dispensary, on-call, weekend, and bank holiday rotas is required.
