Services:

• To

provide a medication review and query services to patients via the telephone or

face to face clinics as appropriate in the practice.

• See

(where appropriate) patient with single or multiple medical problems, including

patients with long term conditions, where medicine optimisation is required.

• Provide

pharmacist support to clinicians attending care home or to attend themselves as

appropriate. Work with care home staff to improve safety of medicines ordering

and administration.

• Managing

caseload of patients with common /minor/self-limiting ailments while working

within a scope or practice and limits of competence. Signposting to community

pharmacy and referring to GPs or other healthcare professionals where

appropriate.
