• Conduct comprehensive medication reviews and optimising medication regimens for patients.

• Provide advice on medication management to patients, healthcare professionals, and the multidisciplinary team.

• Correspond with secondary teams if clarification needed of prescribing plans.

• Deal with all discharge medication reconciliation.

• Support health board prescribing incentive schemes and clinical audits.
