Main purpose of job:

This role provides management and oversight across a range of teams and services, including Social Prescribing, the Mental Health & Wellbeing Integrated Network Team (MINT) and the DHI Community Connector service. These services support individuals to make sustainable changes in their lives through person-centred, preventative and non-medical approaches to health and wellbeing.

The main purpose of the role is to provide effective individual and team management, from recruitment and induction through to ongoing supervision, performance management and development. The role is also responsible for service development, including developing, monitoring and continuously improving services, learning from feedback, good practice and reflection.

The role also has responsibility for stakeholder and contract management, ensuring contractual deliverables and KPIs are met, quarterly reporting requirements are fulfilled, and there is regular communication with commissioners and other key stakeholders.

As part of the Management Team at Southern Brooks, you will contribute to a safe, well-run organisation that puts people and communities at the heart of everything it does. You will build strong relationships with teams across Southern Brooks, external partners and community organisations to support effective services for individuals and communities.

1. People Management & Leadership

• Provide line management, supervision and support to employees across the identified projects, including the Social Prescribing Team Leader, Social Prescribing Link Workers and DHI Community Connector.

• Manage performance, attendance, development and wellbeing, addressing issues appropriately and supporting staff to perform effectively.

• Set the vision and direction for the team and support effective team working and service development.

• Work collaboratively with the team to develop and deliver an Operational Plan aligned with project targets, organisational priorities and service development needs.

• Contribute to organisational strategy, planning, risk management and wider Management Team activity.

• Deputise for the Head of Wellbeing & Health where required.

2. Service & Contract Management

• Oversee the day-to-day delivery and development of the Social Prescribing, MINT Social Prescribing and DHI Community Connector services, ensuring they are safe, effective, person-centred and meet agreed contractual requirements and outcomes.

• Monitor service and contract performance against agreed targets, KPIs and outcomes, identifying areas of underperformance and taking appropriate action.

• Ensure services meet agreed quality standards and organisational policies and procedures and continuously identify opportunities for improvement.

• Develop and maintain effective processes, systems and SOPs to support consistent service delivery and good practice.

• Ensure appropriate safeguarding processes are embedded across services, including recording, reporting and escalation in line with organisational policy and procedures.

• Ensure compliance with relevant governance requirements, including GDPR, health and safety, lone working and other organisational policies.

• Ensure effective recording, monitoring and reporting systems are in place, with accurate and timely information used to demonstrate impact and inform service improvement.

• Manage project budgets with support from senior managers, ensuring resources are used effectively and in line with contractual requirements.

• Build and maintain effective relationships with commissioners, funders, referral partners and other stakeholders, ensuring effective communication and partnership working.

• Develop and maintain effective referral pathways and identify gaps in service provision, working collaboratively with wider Southern Brooks teams to address these where possible.

• Represent and positively promote the services to external stakeholders through meetings, presentations and events.
