As an Experienced Pharmacy Technician, you will play a key role in delivering high-quality, safe and effective medicines management services within the practice. Working collaboratively with GPs, Clinical Pharmacists and the wider multidisciplinary team, you will support medicines optimisation, improve patient outcomes and help ensure prescribing processes are safe, efficient and patient-centred.

Key responsibilities include:

• Supporting medicines reconciliation following hospital discharge, outpatient appointments and other transitions of care.

• Assisting with medication reviews and identifying patients who may require review by a GP or Clinical Pharmacist.

• Managing medication and prescription queries from patients, carers, community pharmacies and healthcare professionals.

• Processing and reviewing repeat prescription requests in accordance with practice protocols and national guidance.

• Undertaking clinical audits, searches and quality improvement projects to support prescribing safety, QOF requirements and contractual obligations.

• Monitoring and actioning medication safety alerts, MHRA guidance, recalls and prescribing updates.

• Supporting the monitoring and recall of patients prescribed medicines requiring regular review or blood test monitoring.

• Liaising with secondary care providers, community pharmacies, care homes and community services to resolve medication-related issues.

• Maintaining accurate and up-to-date clinical records in line with information governance, data protection and medicines management policies.

• Supporting medicines optimisation initiatives, service development projects and the ongoing improvement of pharmacy services within the practice.

• Contributing to the delivery of safe, effective and efficient patient care while promoting best practice in medicines management.
