• Conducting comprehensive medication reviews and optimising medication regimens for patients with complex medical needs.

• Providing advice on medication management to patients, healthcare professionals, and the multidisciplinary team.

• Supporting the management of chronic diseases through medication adjustments and monitoring after discussion with medical team.

• Corresponding with secondary teams if clarification needed of prescribing plans.
