To support the practice with all aspects of medicines optimisation

Patient facing or telephone medication reviews

c.

Structure medication reviews with direct patient contract

d.

Management of long-term conditions

e.

Reviewing systems for safer prescribing

f.

Contributing to repeat prescriptions authorisation and reauthorisation

g.

Actioning acute prescription requests

h.

Contributing to achievement of QOF and locally commissioned quality

improvement schemes

i.

Undertaking clinical audits

j.

Implement prescribing changes

k. Identify national and

local policy and guidance that affects patient safety through the use of

medicines (MHRA alerts, products withdrawals, etc)
