Job description

KEY TASKS AND RESPONSIBILITIES

The post-holder will:

• Work as an integral member of the Glastonbury Health Centre multidisciplinary team, undertaking a patient-facing clinical pharmacy role.

• Support Glastonbury Health Centre in the delivery of relevant requirements of the Network Contract DES, within the scope of the Clinical Pharmacist role.

• Undertake Structured Medication Reviews (SMRs) and medicines optimisation for patients with complex polypharmacy, long-term conditions and other identified medication-related needs.

• Apply clinical expertise to support the safe, effective, evidence-based and cost-effective use of medicines.

• Provide specialist medicines advice and clinical support to patients, GPs and other members of the practice multidisciplinary team.

• Support the identification and proactive management of patients who may benefit from pharmacist intervention.

• Contribute to the management of long-term conditions, within competence and agreed practice and PCN arrangements.

• Support safe prescribing and medicines optimisation through clinical audits, searches, reviews and other quality-improvement activity.

• Support the implementation of relevant national, local and PCN medicines guidance and prescribing initiatives.

• Identify and address medicines-related risks and contribute to patient safety initiatives within the practice and PCN.

• Support appropriate medicines reconciliation and help resolve medication-related issues arising at interfaces between primary, community and secondary care.

• Work collaboratively with community pharmacy, hospital pharmacy and other healthcare colleagues to support continuity and safety of medicines.

• Contribute, where appropriate, to PCN services and initiatives, including Enhanced Health in Care Homes and other relevant Network Contract DES requirements.

• Work collaboratively with other Clinical Pharmacists, Pharmacy Technicians and members of the wider West Mendip PCN multidisciplinary team.

• Develop effective working relationships with relevant ICB medicines optimisation teams and other health and care partners.

• Participate in cross-practice and PCN-wide work where required, recognising that the substantive focus of the post will remain in Glastonbury Health Centre.

• Contribute to service development, evaluation and quality improvement, sharing learning and good practice across Glastonbury Health Centre and the wider PCN.

• Maintain accurate, timely and appropriate clinical records in accordance with professional, practice and PCN requirements.

• Manage their own clinical workload effectively, escalating clinical or operational concerns appropriately.

• Maintain professional competence and undertake the training, supervision and continuing professional development required for an ARRS Clinical Pharmacist working in primary care.

ALL POST HOLDERS ARE REQUIRED TO

• Adhere to relevant West Mendip PCN and host practice policies, procedures and governance arrangements, including those relating to health and safety, equality, confidentiality, information governance and infection prevention and control.

• Maintain their personal and professional development and participate in appropriate training, supervision and development activities relevant to the role.

• Complete all statutory, essential and mandatory training requirements.

• Participate in appraisal and relevant clinical and professional supervision arrangements.

• Maintain confidentiality in relation to patients, colleagues and organisational information and comply with current data protection and information governance requirements.

• Comply with the professional standards, conduct and registration requirements of the General Pharmaceutical Council (GPhC).

• Maintain high standards of infection prevention and control and comply with relevant policies and procedures.

• Maintain complete, accurate and contemporaneous clinical records in accordance with professional, legal, practice and PCN requirements.

• Report incidents, complaints, clinical concerns, safeguarding concerns and other risks through the appropriate practice and PCN processes.

• Take responsibility for the quality and safety of their own work, working within their scope of competence and seeking advice or escalating concerns where appropriate.

• Contribute positively to the effectiveness of the Glastonbury Health Centre and West Mendip PCN teams, including identifying opportunities to improve services and patient outcomes.

• Work effectively and respectfully with colleagues across practices and other health and care organisations to meet patients' needs.

• Manage their own time, workload and resources effectively.

NOTE

This job description provides an outline of the principal responsibilities of the post and is not intended to be exhaustive.

The post is an ARRS-funded West Mendip PCN role predominantly aligned to Glastonbury Health Centre. While the post-holder's principal day-to-day responsibilities will be undertaken with Glastonbury Health Centre, flexibility will be required to work collaboratively across West Mendip PCN and with partner organisations where appropriate to meet patient, service and Network Contract DES requirements.

The duties and responsibilities of the role may evolve in response to changes in national guidance, the Network Contract DES, local priorities and service needs. Any significant changes will be discussed with the post-holder.

All duties must be undertaken in accordance with applicable West Mendip PCN, host practice and professional policies, procedures and standards.
