Job description

Main Duties of the Job

In this role, you will:

Support the Partners in implementing effective medicine management and patient care within the practice/s.

Identify areas for improvement and managing change

Deliver patient-facing clinical care: Conduct consultations, prescribe medication, run clinics (especially for long-term conditions), and support vulnerable or complex patients, including home visits.

Enhance patient understanding and outcomes: Educate patients and carers on medication use, improve adherence, and provide ongoing support to build confidence in treatments.

Ensure safety and quality of prescribing: Monitor high-risk drugs, handle medication incidents, carry out audits, and align practice with clinical standards (e.g., NICE) to improve safety and performance.

Collaborate, train, and develop services: Work within the multidisciplinary team, support staff training, liaise with external stakeholders (e.g., community pharmacists), and contribute to service improvement and professional development.

Improve medicines management systems: Oversee prescribing processes, medication reviews, repeat prescriptions, and ensure safe, effective, and high-quality use of medicines across the practice
