Job description

Job Purpose We are seeking an experienced and motivated Clinical Pharmacist Independent Prescriber to join our multidisciplinary team.

The successful candidate will work independently whilst having the support of an experienced clinical team. They will lead on medicines optimisation, structured medication reviews, prescribing safety and management of patients with long-term conditions. The post-holder will play a key role in supporting the delivery of the NENC Medicines Local Enhanced Service, including shared care prescribing, injectable medicines services and oral antipsychotic monitoring.

Main Duties and ResponsibilitiesMedicines Optimisation

• Undertake structured medication reviews and clinical medication reviews.

• Manage complex polypharmacy and high-risk medicines.

• Support prescribing safety and medicines optimisation initiatives.

• Review pathology results related to medicines monitoring.

• Reconcile medicines following hospital discharge.

• Respond to medicines-related queries from patients and clinicians.

• Support implementation of NICE guidance and local prescribing policies.

Independent Prescribing

• Assess, diagnose and prescribe within scope of competence.

• Manage patients with long-term conditions through face-to-face, telephone and digital consultations.

• Support the management of patients with complex medication needs.

• Work collaboratively with GPs and the wider clinical team to improve patient outcomes.

Shared Care and LES Delivery

• Lead on Shared Care prescribing and monitoring in line with NENC and NTAG guidance.

• Manage patient recalls and monitoring requirements for shared care medicines.

• Support existing ADHD shared care patients where prescribing responsibility has been accepted by the practice.

• Maintain accurate coding, registers and documentation required for LES monitoring and reporting.

• Support delivery of injectable medicines services and annual oral antipsychotic reviews.

Quality Improvement

• Undertake prescribing audits and quality improvement projects.

• Support the implementation of MHRA safety alerts and prescribing guidance.

• Identify opportunities to improve prescribing quality, safety and cost-effectiveness.

• Contribute to practice and PCN medicines optimisation initiatives.

Education and Team Support

• Act as a source of expert medicines advice for the practice team.

• Provide education and prescribing support to GPs, nurses and other healthcare professionals.

• Support the development of safe and effective prescribing across the practice.
