Job description
Duties
and responsibilities
There may be queries with some
requests, some may be acute. Training
will be given to establish who to direct the query to.
Add medication from discharge letters
and clinical letters to patients electronic record. Refer to the Practice
Pharmacist when appropriate
To provide first point of contact for patients
requesting repeat medication
Liaise with Community Pharmacists
regarding vulnerable patients

Working Relationships
To work as a flexible
member of the team to ensure the smooth running of the practice, providing
support to other team members when necessary

undertaking similar work
Training may need to be undertaken outside
of normal practice hours, and off site

quality and risk
Assess own performance and take
accountability for own actions, either directly or under supervision
Contribute to the effectiveness of the team
by reflecting on own and team activities
and making suggestions on ways to improve and enhance the teams

agencies to meet patients needs
Effectively manage own time, workload and
resources
Communication
The post holder should recognize the

Identifying the risks involved in work
activities and undertaking such activities in a way that manages those risks

knowledge and skills
Using appropriate infection control
procedures, maintaining work areas in a tidy and safe way and free from hazards
