Job description

• Conduct structured medication reviews (SMRs) for patients with complex polypharmacy, frailty, or long-term conditions.

• Manage and monitor long-term conditions (e.g., hypertension, asthma, diabetes) under appropriate clinical supervision.

• Provide face-to-face, telephone, and digital consultations.

• Support safe deprescribing and medicines reconciliation following discharge or clinic visits.

• Implement national and local guidelines to ensure evidence-based prescribing.

• Undertake clinical audits and quality improvement initiatives.

• Identify and manage medicines-related risks, safety issues, and MHRA alerts.

• Support antimicrobial stewardship initiatives.

Review repeat prescriptions, ensuring accuracy, appropriateness, and compliance.

• Oversee repeat prescribing processes and medication synchronisation.

• Support medicines optimisation, cost-effective prescribing, and formulary adherence.

• Assist with shared-care agreements and high-risk medication monitoring.
