Responsibilities

Collaborate

with pharmacists and other healthcare professionals to develop and implement

personalised medication therapy plans.

Resolve

medication related problems for patients and support medication management and

counselling

Manage

patient profiles, including medication history, allergies, and current

prescriptions

Provide

patient education on medication use, potential side effects, and drug

interactions.

Assist in

quality improvement initiatives, including medication error prevention

Conduct

medication reconciliation and identify discrepancies to ensure accurate and

up to date medication lists.

Participate

in multidisciplinary meetings to discuss patient care and optimise treatment

outcomes.

Stay up

to date with current literature and guidelines to provide evidence based

pharmaceutical care.
