Job description
Main Duties and Responsibilities
Leadership
• To propose and comment on policy and service changes and plans within the pharmacy service.
• To contribute to, and participate in, pharmacy service developments, and co-ordinate and implement them in line with departmental and Trust policy.
• To ensure services provided are in line with national agendas, policies, guidelines and frameworks.
• To actively participate in and contribute to the development, promotion and local implementation of local and regional prescribing guidance.
• To work with the Deputy Chief Pharmacist, Operations to strategically develop pharmacy services
• To lead on specific projects or initiatives across all of the Pharmacy Service
• To motivate and inspire others (both within pharmacy and the Trust).
• To demonstrate innovation.
• To extend the boundaries of the profession.
• To work collaboratively with senior pharmacy and medical staff, the multidisciplinary team and management and to participate in the development of pharmacy services in accordance with the agreed objectives.
• To develop, implement and monitor policies, guidelines, protocols and procedures at Trust and regional level, as required.
Clinical Responsibilities
• To act as a specialist practitioner within the field and as an expert pharmacist in the Care Group.
• To promote and deliver pharmaceutical care for Care Group patients and more generally for all patients.
• To provide highly specialised and complex evidence-based clinical advice and information using expert knowledge and experience to pharmacy, nursing, medical and other healthcare professionals on the care and treatment of patients, inside and outside of the Trust.
• To participate in and contribute to the clinical/ward pharmacy service.
• To actively participate in consultant ward rounds as appropriate.
• To participate in outpatient clinics, and assist in the further development of the pharmacists role in this area.
• To undertake patient medication review sessions and patient reviews in outpatient clinics as necessary.
• To actively participate regularly in multidisciplinary ward rounds, antibiotic ward rounds and other multidisciplinary meetings as appropriate, delivering and promoting high quality, cost-effective, patient-focused pharmaceutical care.
• To practice as an independent prescriber, in area of own expertise, as appropriate and provide direct high quality care to inpatients and outpatients within scope of practice
• To participate in patient education and counselling schemes for all Care Group patients/carers.
• To ensure appropriate and timely supply of medication to patients in conjunction with dispensary staff, including the POD scheme and discharge facilitation for patients.
• To liaise with other Pharmacists, Doctors and GPs to ensure efficient transfer of clinical and medicines management issues.
• To develop shared care guidelines to support GP prescribing as necessary.
• To support the set up and provision of Hackett compliant homecare services as necessary.
• Maintain and further develop specialist information on the use of drugs in clinical conditions relevant to the Care Group, ensuring accurate and up to date clinical information is available to all pharmacists performing out of hours duties in a format relevant to those pharmacists needs.
• To participate in Trust adverse incident reporting scheme
Project Management & Policy Development
• To provide expert project management skills to support Pharmacy operations and medicines management processes across various clinical/ specialty areas.
• To provide direction to other project stakeholders and managers to guide them through initiatives or projects being delivered, ensuring plans are maintained and updated consistently.
• To work with key stakeholders to develop, refine, and champion project management best practice.
• To provide expertise on project structuring, planning, benefits quantification and tracking on behalf of Pharmacy.
• To analyse and resolve highly complex problems to successful conclusion.
• To manage the creation and delivery of the project plan, including milestones, benefits, risks, approach and resource requirements.
• To be responsible for managing individuals and teams, which will support the delivery of the project.
• To be prepared at any point in time to provide a detailed overview of the key project milestones including associated dependencies, risks and issues.
• To be responsible for policy development required to support service changes during the transformational process.
• To identify interdependencies and constraints that impact project delivery.
• To manage and escalate project risks/ issues as needed maintaining a risk and issues register.
• To provide subject matter expertise towards the development of best practice methodologies, policies and strategies.
Information Management
• To ensure there are effective systems of governance within function and role.
• To ensure that data used to support service improvement has the appropriate checks before being used and ensure that any data quality issues are fed back to the appropriate teams.
• To communicate clearly the information requirements of projects and use analytical skills to articulate requirements and present end product as needed.
Communication & Relationship Management
• To build and maintain good working relationships with a broad range of internal and external stakeholders on a range of programme and policy issues.
• To lead as the expert; integrating systems and managing effective working relationships with the appropriate stakeholders.
• To provide and receive complex, sensitive and contentious information, including presenting information about projects and dependencies to a wide range of internal and external stakeholders in formal settings.
• To manage potentially difficult situations with staff and stakeholders within change programmes for successful outcomes.
• To deal with complex and conflicting problems and day to day work load in workshops, meetings, one on one communications and other events, comprising various parts of the business.
• To nurture key relationships with senior individuals who can influence policy
• To link with managers and members of other initiatives to address inter-dependencies and ensure alignment.
• To use highly developed leadership and influencing skills with the ability to enthuse, motivate and involve stakeholders and teams across the Trust, such that they understand and work to achieve the Trusts performance expectations.
• To ensure optimum engagement from staff, colleagues and stakeholders in order to deliver continuous change, securing appropriate buy in, support and understanding and effective flows of information.
• To write and deliver communication strategy for the project as appropriate.
• To ensure project groups/ forums are furnished with appropriate project documentation to agreed time scales.
Financial Management
• To ensure the effective business justification is presented for allocation of any required capital funding or increase in revenue budgets, developing and presenting business cases to appropriate sub-committees where necessary.
• To hold accountability and sign off for all projects delivered
• To constantly strive for value for money and greater efficiency in the use of resources and ensure that they operate in recurrent financial balance year on year.
• To ensure physical/ people resources are deployed within the project financial envelope appropriately and ensure that any physical devices are issued and returned.
People Management/Performance
• Transfer expertise and knowledge as appropriate, developing and delivering formal briefing and training to promote innovation in pharmacy and multidisciplinary teams.
