Job description

The post holder is a Clinical Pharmacist who will work within their professional boundaries to provide a high-quality clinical pharmacy service across Erewash Primary Care Network. The successful candidate will work collaboratively with GP practices within the network to deliver patient-focused medicines optimisation, improve patient outcomes, and support the safe and effective use of medicines.

The post holder will work alongside other Clinical Pharmacists and the wider multidisciplinary team, including GPs, Advanced Clinical Practitioners, and other healthcare professionals. They will be supported by Community GPs within member practices and a Senior Clinical Pharmacist who will provide mentorship, clinical support, and opportunities for professional development.

The role will involve undertaking structured clinical medication reviews, supporting long-term condition management, addressing medicines-related queries, and proactively managing patients, including those with complex polypharmacy and multiple long-term conditions.

The post holder will provide expert support to practices with prescription and medication queries, supporting safer prescribing systems, medicines reconciliation following transfer of care, repeat prescribing processes, and acute prescription requests. They will provide clinical medicines advice to patients and healthcare professionals, helping to improve access to pharmacy expertise within primary care.

The post holder will contribute to medicines optimisation, quality improvement initiatives, and delivery of PCN priorities, including supporting aspects of the Quality and Outcomes Framework (QOF) and enhanced services.

The role will also support closer integration between general practice, community pharmacy, and secondary care services, helping to maximise skill mix, improve patient outcomes, and enhance the efficiency and quality of medicines-related processes.

Experience of working with older people, frailty services, or within an Ageing Well model would be desirable, with the opportunity for the successful candidate to develop a specific frailty-focused role within the PCN. This may include supporting proactive care for older adults with frailty, complex needs, and medicines optimisation challenges.

The role is pivotal in supporting the continued development of the PCN clinical pharmacist service and requires an enthusiastic, motivated pharmacist with a passion for delivering excellent patient care within a forward-thinking primary care environment.
