Hybrid model including 2 practice-based

days, 1 hub day and 1/2 remote working days.

Medicines reconciliation following hospital

discharge

Responding to prescribing and medication

queries

Supporting repeat prescribing and medication

safety processes

Assisting pharmacists with medication

reviews and structured medication reviews

Supporting audits and medicines optimisation

initiatives
