Job description

As a valued member of our multidisciplinary team, you

will work closely with Clinical Pharmacists, GPs, nurses, administrators, and

other healthcare professionals within and outside of the organisation to ensure

the safe, effective, and cost-efficient use of medicines. You will support a

range of activities including medicines reconciliation, new patient medication

reviews, prescription management, responding to medication queries, and

identifying opportunities to improve patient outcomes in areas such as cardiovascular

disease prevention. Another key area is supporting with the management/care of

our patients with hypertension, through direct patient contact, up titration of

medication and lifestyle counselling.
