Key responsibilities include:
• Supporting medicines optimisation and safe prescribing across the Partnership
• Undertaking medicines reconciliation following hospital discharge and other care transitions
• Supporting repeat prescribing processes and medication reviews
• Assisting with structured medication reviews and high-risk drug monitoring within your competency
• Responding to pharmacy-related patient enquiries and supporting medicines advice services
• Completing prescribing audits and contributing to quality improvement initiatives
• Working collaboratively with Clinical Pharmacists, GPs, prescribing teams, and pharmacy administrators
• Maintaining accurate clinical records and ensuring compliance with prescribing policies and procedures
• Using digital systems and innovative technologies to improve patient access and prescribing efficiency
• Contributing to the ongoing development of the Pharmacy Hub and new models of care
