Job description

Job Summary

The Clinical Pharmacist will work as part of the multidisciplinary general practice team to improve the quality, safety and cost-effectiveness of prescribing and medicines optimisation. The postholder will undertake medication reviews, support patients with long-term conditions, provide specialist pharmaceutical advice, and contribute to delivering high-quality, evidence-based patient care.

Where appropriately qualified, the postholder will independently assess, diagnose and prescribe within their scope of competence.

Key Responsibilities

Clinical Responsibilities

• Undertake structured medication reviews.

• Optimise medicines for patients with long-term conditions.

• Identify and reduce medicines-related risks.

• Review polypharmacy in complex patients.

• Support care home prescribing.

• Undertake medicines reconciliation following hospital discharge.

• Provide pharmaceutical advice to clinicians and patients.

• Manage medication queries from patients and healthcare professionals.

• Support safe repeat prescribing processes.

• Promote antimicrobial stewardship.

• Assist with medicines safety alerts and recalls.

Independent Prescribing (where qualified)

• Conduct patient consultations.

• Assess, diagnose and prescribe within scope of competence.

• Manage long-term condition clinics.

• Monitor treatment effectiveness.

• Refer appropriately when necessary.

Medicines Optimisation

• Improve prescribing quality using evidence-based guidance.

• Support implementation of NICE guidance and local prescribing policies.

• Participate in clinical audit and quality improvement projects.

• Review prescribing data and identify opportunities for improvement.

• Support achievement of Quality and Outcomes Framework (QOF) and other contractual requirements.

Practice Support

• Advise clinicians on prescribing issues.

• Develop prescribing protocols and guidelines.

• Deliver educational sessions to practice staff.

• Contribute to multidisciplinary team meetings.

• Work collaboratively with community pharmacy, secondary care and other healthcare providers.

Administration

• Maintain accurate contemporaneous clinical records.

• Ensure prescribing documentation meets professional standards.

• Participate in practice meetings.

• Assist in developing pharmacy services.
