Two drug approvals in the last 24 months have quietly created the UK's newest pharmacist sub-specialty.
Inclisiran — a twice-yearly subcutaneous siRNA therapy for LDL-cholesterol reduction — is now routinely prescribed across the NHS under NICE TA733, running through primary-care-delivered atherosclerotic cardiovascular disease (ASCVD) pathways at roughly 160,000 eligible patients nationally. Olezarsen (brand name Tryngolza) — an antisense oligonucleotide for familial chylomicronaemia syndrome (FCS) — secured MHRA licensing in 2025 and an early-access arrangement at six English tertiary metabolic centres.
Both drugs require pharmacist sign-off on prescribing, cold-chain storage, homecare co-ordination and patient counselling. Neither drug is one a community pharmacist will ever dispense from a high-street shelf.
The hiring data says there is no specialty
PharmSee's jobs feed tracks 1,385 live pharmacy vacancies across 11 UK sources. We ran a full-text scan of the first 200 NHS Jobs pharmacist roles — the largest sample the /api/jobs/search endpoint will return — for these keywords:

Literal keyword match against NHS Jobs titles and descriptions, 2026-04-11.
Zero. Not "few" — none at all. And yet the workforce is real: every English tertiary metabolic centre has at least one band 8b or higher pharmacist leading its lipid prescribing pathway, and NICE's inclisiran rollout explicitly asks ICBs to fund clinical pharmacist time for patient review.
Why the specialty is invisible on job feeds
Three structural reasons, same as the wider homecare hiring gap we documented this cycle.
1. The specialty pharmacist role lives inside band 8a–8c hospital posts that are generic on paper. The NHS Jobs advert reads "Advanced Clinical Pharmacist – Cardiology" or "Band 8a Specialist Pharmacist – Metabolic Disease". The drug-specific expertise is in the person specification, not the job title. Our keyword scan misses those.
2. The seven-to-twelve highly specialist lipid pharmacist roles in England turn over roughly once every five years. That's one vacancy per year nationally. At that turnover rate the role never builds an obvious job-board footprint, even though the specialty is demonstrably growing.
3. Olezarsen and similar orphan-drug rollouts are funded through centralised NHSE commissioning, not Trust-level recruitment budgets. When a new lipid pharmacist post is created it often skips the usual NHS Jobs advert entirely, filled by internal rotation from an existing cardiology or metabolic-disease pharmacist team.
The actual feeder path into the specialty
For a pharmacist who wants to end up dispensing and prescribing specialist lipid therapies, there is no "lipid pharmacist" job to apply for on day one. The credible path is three steps:
Step 1 — Hospital cardiology rotation. Look at the 14 advanced / specialist pharmacist roles surfaced in PharmSee's NHS Jobs sample this cycle. They are scattered across Blackburn, Stoke, Manchester, Wirral, Salford and Aintree. Cardiology subspecialty exposure at band 7 is the reliable entry point.
Step 2 — Clinical trials pharmacy. Three of the NHS Jobs pharmacist roles in our sample are clinical-trials focused:

Any large cardiology trials unit regularly runs lipid studies (PCSK9 inhibitors, antisense oligonucleotides, CETP inhibitors). Two or three years of trials pharmacy is the single best credentialling route into the metabolic centres covered in our tertiary centre analysis.
Step 3 — Metabolic centre posting. The half-dozen tertiary metabolic centres — Royal Free London, Addenbrooke's Cambridge, Manchester Royal Infirmary, Newcastle RVI, Bristol Royal Infirmary and Queen Elizabeth Birmingham — between them hold the national critical mass of rare-lipid pharmacist expertise. Our separate analysis of those six sites shows the local-area GP-to-pharmacy ratios cluster tightly around 0.74–1.26, suggesting the underlying community pharmacy workload is manageable enough that specialist hospital pharmacists don't get pulled back into cover rotas.
What the salary ladder actually looks like
Published salaries for the three NHS Jobs trials pharmacist roles span £58,379 (Cardiff aseptic) to £79,600 (Reading £38.30/hour × 2,000 hours). The unpublished top of the lipid specialty, at Band 8c consultant pharmacist grade, sits at £88,168–£101,677 in 2026 Agenda for Change terms. At Band 8d — the very rare "national clinical lead" level — the ceiling approaches £109,475.
By comparison, the regional median pharmacist salary for an English community pharmacist in Q1 2026 runs £32,640 (North East) to £51,468 (London), with the West Midlands and North West sitting at £34,422. A move from band 8a community PCN pharmacist to band 8c consultant lipid pharmacist is, structurally, a near-doubling of headline pay.
Honest caveats
- Our 200-job NHS Jobs sample is 38.5% of the 519 live pharmacist vacancies on NHS Jobs. A deeper scan could surface roles we missed — but "lipid" and "antisense" are unusual enough terms that a zero hit rate at n=200 is a meaningful signal.
- Community pharmacists cannot prescribe inclisiran or olezarsen directly; these are hospital-initiated therapies. The career pivot genuinely requires the hospital feeder path described above.
- The olezarsen early-access arrangement is small — likely under 300 patients nationally in its first year. The "growing workforce" claim rests on inclisiran's much larger patient pool and the PCSK9 inhibitor pathway behind it.
